States will have option to provide "health homes" for people with chronic conditions

The Centers for Medicare and Medicaid Services (CMS) issued guidance to state Medicaid directors on the implementation of section 2703 of the Affordable Care Act, "State Option to Provide Health Homes for Enrollees with Chronic Conditions." This will allow states to choose to offer, under their Medicaid state plan, coordinated care through a health home for eligible individuals with chronic conditions. If implemented, states will receive a 90% matching rate for the enhanced integration and coordination of primary, acute, behavioral health (mental health and substance use), and long-term services and supports for persons with chronic illness.

The guidance letter also includes instructions for states that already conduct home health initiatives. Section 2703 will go into effect January 1, 2011.
